What Is Myofascial Release Therapy — And Why Does It Work?

It holds everything together — providing structure, transmitting force, and allowing movement. When it is healthy and pliable, you barely notice it. What Myofascial Release Actually Is
Myofascial release is a hands-on manual therapy technique that applies gentle, sustained pressure to areas of fascial restriction. Unlike deep tissue massage, which targets muscle belly directly, myofascial release works with the connective tissue itself — holding pressure at specific points and waiting for the fascia to soften and release.
The sensation is distinct. Most patients describe it as a deep but relieving pressure — like feeling a knot finally let go. The relief is often immediate. The long-term effects, delivered through a course of treatment, are lasting.
How Usha Brought It to Fiji
In the early 1990s, Usha Krishna-Gfeller was working as a physiotherapist in the United States when she trained in myofascial release — at a time when the technique was still relatively new to clinical practice. She recognised immediately that this was different. Better. She returned to Fiji and built it into the very foundation of her clinic's treatment philosophy.
That was over 30 years ago.
Today, every physiotherapist at Physiotherapy Clinic Fiji has been personally trained in myofascial release by Usha. It is not an add-on or an optional extra. It is the core of how we treat every patient who walks through our doors.
In a standard 40-minute session, at least 25 to 30 minutes will involve active, hands-on myofascial work. No machine. No passive electrotherapy. Skilled hands — every session, every patient.
Who Benefits From Myofascial Release
The honest answer is: most people with a musculoskeletal condition will benefit to some degree. The technique is particularly effective for:
Back pain and neck pain — especially chronic or postural. Tension headaches and migraines. Frozen shoulder and rotator cuff restrictions. Sports injuries and post-surgical stiffness. What to Expect in Your First Session
Your first appointment begins with an assessment. Your physiotherapist will ask about your history, symptoms, and what you have already tried. They will then examine the area of restriction — feeling for tightness, asymmetry, and areas that are guarded or tender.
From there, treatment begins. You will feel sustained pressure applied to specific points. Some areas will release quickly. Others — particularly if long-standing — may take several sessions to work through. Most patients notice a meaningful improvement in mobility or reduction in pain after the first one or two sessions.
